Biography :

My passion has been to optimize the well-being of elderly patients by introducing frailty screening tools at the Emergency Department and community nurse posts, with subsequent multi-disciplinary interventions within the hospital and with community partners’ collaborations. These are with the intention to right site care with consequent reduction of emergency department reattendances and hospitalisations from complications secondary to frailty. Another service I started is the outreach to nursing homes(NH) to upskill and support NH staff so as to right site care within their premise. The objective is to reduce unnecessary and avoidable acute hospital utility, whether in the form of emergency department (ED) usage or hospitalisations
